- Download Membership Application Form ( PDF )
- Mail the completed form^ and entrance fee* by a cheque (made payable to “Hong Kong Public Hospital Cardiologists Association Limited”) to Flat A2, 29/F, One Soho, 32B Shantung Street, Kowloon (Attn : Ms. Wandi LAI).
- Your application will be reviewed by the Council, subject to approval.
* Entrance fee: $300 for full member (Registered Specialist in Cardiology, HK), $200 for associate member (Higher Physician Trainee in Cardiology, HK)
^ Only full member is eligible to vote in general meeting. Proposer and Seconder shall be full member of HKPHCA.
