- Download Membership Application Form ( PDF / DOC )
- Mail the completed form^ and entrance fee* by a cheque (made payable to “Hong Kong Public Hospital Cardiologists Association Limited”) to Flat F, 16/F, Block 3, Bellagio, Sham Tseng, NT (Attn : Ms. Wandi LAI).
- Your application will be reviewed by the Council, subject to approval.
* Entrance fee: $300 for full member, $200 for associate member
^ Only full member is eligible to vote in general meeting. Proposer and Seconder shall be full member of HKPHCA.
